Click on My Account in the upper right hand corner of glidewelldental.com |

Step 2 |
|
If you have an existing account with Glidewell Laboratories, and you have registered for online access to My Account, Log In using your Account Number and Password.
If you do not already have a Glidewell Account or would like to register your Glidewell Account for online access, please call 800-854-7256. |

Step 3 |
|
Once logged in, you should be able to view your Account Summary.
In the Main Menu, Click on Upload Digital Files, then click on CEREC Files. |

Step 4 |
|
You should now see the CEREC Online Rx. Begin by completing the required Patient Infomation fields. |

Step 5 |
|
Select your Restorations one at a time (page will refresh after each selection). First, click on the restoration type. Then click the box next to the tooth number to apply the restoration type to the tooth.
You should see a letter in the check box indicating the restoration type has been successfully applied.
Continue to apply restoration types to each tooth number as needed. |

Step 6 |
|
As each restoration type is applied to each tooth number, a chart is generated with additional required product information.
For each tooth number, indicate whether you only need a CAD Design or complete the required shade, staining and product information.
Click Next. |

Step 7 |
|
To attach the appropriate CEREC file to the prescription, click on Browse and use your standard computer dialogue to find the CEREC file on your computer.
The file path should appear in the field next to the Browse button. Click Attach.
If necessary, click on expand to upload additional files for this patient.
You will see your CEREC file(s) listed under "Attached Files" if you have attached them successfully. |

Step 8 |
|
Type in any special instructions needed for the restoration to be completed properly.
Click on Prescribing Doctor's Office or Laboratory to bring up the proper Authorization dialogue. |

Step 9 |
|
Fill out the appropriate information and select a delivery date (Doctor form shown here).
Click Next. |

Step 10 |
|
You should now see a Summary of your prescription. If you need to edit the information you may click Back to navigate to the appropriate section. You may also start over by clicking the Start Over button.
If you are satisfied that the information is correct, click on Finish to submit your prescription to us. |

Step 11 |
|
You should now see a Confirmation / Case ID number and a link to print the Summary of your prescription. Please print and save a copy of the prescription for your records.
Click on Finish to complete the Rx. |
